Please call us early and fill out the order form and return it to us. This allows us to secure your date on our calendar. If we have not previously worked together, please telephone us for details. We can discuss your needs, dates, and prices.

This agreement is between Elsenpeter Productions and:

Please attach map or directions to this agreement before mailing. It will be inevitable from time to time that a show will have to be rescheduled, due to an act of God such as illness, family problems, weather, accident, etc. We will notify you of any change that will have to be made and every effort will be made to reschedule it for a time that is mutually convenient to all.

Agreement is subject to the terms and conditions set forth on the bottom of this agreement.

Representative will ensure that payment is sent directly to Elsenpeter Production. PLEASE DO NOT PAY THE PERFORMER. Payment will be sent to the above listed address.

Representative understands that they are obligated to pay for services of Performer. In the event of fire, accident, flood, sickness, riots, strike, car problems, acts of God or other causes beyond control of Performer or Representative which prevents the aforementioned performance, the parties hereto release each other from any and all performance of the terms and conditions herein contained and from damages resulting from the breach thereof. In the event of cancellation, the performance will be rescheduled for a later date.

This is a non-cancelable booking agreement between the parties whose signature are affixed.

No performance shall be recorded, reproduced or transmitted in any matter without specific written permission from Elsenpeter Production.

It is hereby agreed that the undersigned Performer is an independent contractor and not an employee of the Elsenpeter Production.

The Representative agrees to be responsible for harm, loss or damage of any kind to the performer(s) or property of performer while located at place of performance.

This contract shall be governed by the State of Illinois.

This contract contains the entire agreement between the Representative and Elsenpeter Productions and no statement, promise or inducement made by either party that is not contained in this written contract shall be valid or binding and this contract may not be enlarged, modified or altered, except in written signature by both parties endorsed hereon.
